New Patient Referral
Frequently Asked Questions
To be referred to our center, your primary care physician, specialist, or healthcare provider must submit a formal referral. This can be sent via fax, email, or through our secure referral portal. Once we receive and review it, our team will contact you to begin the appointment process.
In most cases, a referral is required to book a new patient appointment, especially if your insurance policy mandates specialist referrals. This ensures that we have the proper clinical information before your visit. Please get in touch with your insurer or our staff to confirm your specific requirements.
Referrals should include a detailed reason for referral, recent imaging or lab results, relevant medical history, and the referring physician's contact details. This information helps our specialists understand your case better. It also streamlines the appointment and diagnostic process.
After we receive your referral, our scheduling team will review your information and reach out to you for an appointment. You can also contact our office directly to check on the status. We aim to provide timely access based on medical urgency and provider availability.
For your first visit, please bring an insurance card, a valid photo ID and a list of current medications. This documentation is essential for a comprehensive evaluation. Bringing everything ensures your appointment runs smoothly.
You can confirm the receipt of your referral by calling our referral coordinator at (910) 491-2744 ext 108. We will notify you once your referral is reviewed and an appointment is ready to be scheduled. Your referring provider may also receive confirmation.
